Remove all uses of selector &

Browse Source 
If we want to support namespaced import:

```scss
.twbs {
  @import 'bootstrap';
}
```

We cannot use `selector &`, see sass/sass#1817.

`fieldset[disabled] &` is not needed as we no longer support IE8.
