Some places where using the wrong icon and now that they have changed,
they need to be updated. For instance:
- i/settings (cog) should be used for settings or configure.
- t/edit (pen) should be used for editing

MDL-73347 blocks: New unaddableblocks theme setting
...
A new method, get_unaddable_by_theme_block_types(), has been added to
block_manager class to let themes define, using the unaddableblocks
theme setting, the blocks that won't be displayed in the "Add a block"
list.
For now, only the boost theme uses it, to disable some of the blocks,
like settings, navigation, course_list and section_links (they are still
required for classic).

This was broken by changes in fdd5aef0, as the sesskey URL parameter
was required on admin/index.php in order for block related actions
to be performed uninterrupted by any redirections.

MDL-60174 core_dml: fix miscellaneous incorrect recordset usage

The new recordset support for Postgres requires transactions and
will cause errors if recordsets are not closed correctly. This
commit fixes problems that were identified during unit tests, and
via some basic code analysis, across all core code. Most of these
are incorrect usage of recordset (forgetting to close them).
