Allows plugins to add columns to the question bank view by extending core_question\bank\column_base
Columns to display are set in $CFG->questionbankcolumns. Columns are namespaced and autoloaded to support this.
The symptom is that where we are now is not shown in the settings
navigation. However, the underlying cause is that question/edit.php is
doing some crazy thing of its own to build $PAGE->url, rather than using
the one returned by question_edit_setup.
Not that this patch itentionally removes the returnurl param. It was
added as part of a big change MDL-20276, which I think just got this
detail wrong. That variable is never used.

This is a big change, and the result is not perfect yet. Expect some debugging output
on some pages.
The main part of these changes are that $OUTPUT->header now looks for a file
in the theme called layout.php, rather than header.html and footer.html. Also
you can have special templates for certain pages like layout-home.php. There is
fallback code for Moodle 1.9 themes, so they still work.
A few of the old arguments to print_header are no longer supported. (You get an
exception if you try to use them.) Sam H will be cleaning those up.
All the weblib functions that have been replaced with $OUTPUT-> have version in
deprecatedlib, so existing code will go on working for the foreseeable future.
That allows us to have a few sentences explanation of each question type, and is also easier to fit into a tight layout.
This commit should also fix MDL-18214 Layout screwed in Safari with long category names.
Step 1. Just convert the existing functions quiz_question_showbank and question_showbank and friends into methods of a class and subclass.

2. Converts the truefalse question type to formslib.
