MDL-78806 core: Remove redundant site name on page titles
* With the site name now being appended to the page titles, there
is no need to manually append the page titles.
Create or update the breadcrumbs in the site administration
pages where it is required.
Highlight the corresponding site adminstration tab.
Highlight the primary nav to Site administration when user
is navigating to any of the site administration pages.
Also changed the boostnavbar so that the nodes in the secondary
navigation are not shown in the breadcrumbs when user is in site
administration page.
useredit_update_picture as moved to user_update_picture
as it's more general. It was also moved to user/lib.php
so it can be used by both webservices and edit without more include files.
Preferences pages cancel and continue buttons should lead back to preferences page.
Site pages should have consistent headers and breadcrumbs.
Hide the "Preferences" node in the navigation.

This patch replaces all homegrown timezone
stuff with standard PHP date/time code.
The main change is the introduction of core_date
class that returns normalised user and server
timezones. From now on nobody should be using
$CFG->timezone or $user->timezone directly!
Other new features and fixes:
* admins are prompted for timezone during install
* editing of other users is finally fixed
* timezones are displayed in user profile
* new $this->setTimezone() in phpunit
* time locale is now automatically reset in phpunit
* timezone is now automatically reset in phpunit
* phpunit has Australia/Perth as default timezone
user_updated event was being triggred while
setting password, which is not correct. It now
trigger user_password_updated event. Few more
modifications done:
1. Correct event is being triggred.
2. Event is only triggred when password is chnaged.
3. Password is updated via single api.
4. Updated unit test
On some pages full user data is not stored before user_created/
user_updated event is triggred. So giving option to trigger that
manually, so it can be triggred after all data is saved
List of changes:
* New OOP API using PHP namespace \core\session\.

* Other minor bugfixing and improvements.
* Fixed database session segfault if MUC disposed before $DB.
Limitations:
* Session access time is now updated right after session start.
* Support for $CFG->sessionlockloggedinonly was removed.
* First request does not update userid in sessions table.
* The timeouts may break badly if server hosting forces PHP.ini session settings.
* The session GC is a lot slower, we do not rely on external session timeouts.
* There cannot be any hooks triggered at the session write time.
* File and memcached handlers do not support session lock acquire timeouts.
* Some low level PHP session functions can not be used directly in Moodle code.
GD PHP extension is now required. Add-ons need to remove $CFG->gdversion tests. The worst case regression is that add-on will think GD is not available.
This patch includes:
* user management suspend UI tweaks and other coding style improvements
* suspended info in user profiles
* suspend option in advanced edit form
* suspended user filtering
* hidden user fields option for suspended field

The new messaging default settings can be set in messages.php and deployed
during installation. This also ensures the removing of settings on plugin
uninstallation and contains the update script to populate current default
settings on the existing system when the new feature is introduced.
For security reason we have to avoid using library functions in upgrade
function, so we set defaults the blind way. At this point we do not expect
plugins to have individual messaging defaults presets anyway. The site
defaults are the same as were set for each user using
message_set_default_message_preferences function.

This patch fixes incorrect password creating, updating and resetting, updating of user fields, unsupported auth plugins are correctly identified, modification of mnethostid is prevented, fixed problem with email duplicates, new password is generated for users without email, etc. It also includes coding style improvements, more inline docs, future TODOs and license information.
