I thought about renaming the class to just plain attempt, but I
acutally think quiz_attempt makes it clearer what this is. Also
not changing the name massively reduces the pain for everyone
updating their code (including me right now!)
This got lost in the 4.0 question bank work.
We are trying to phase out qtype_random, that is where the old
code and lang strings for describing random questions is, so
OK to add back a call to it. When we get better qbank filtering
we can replace this again.
Note, the asserts in the Behat were copied from my 3.11, checkout,
to really verify we are restoring the previous behaviour.
I think that, because how PHP works, the MDL-74255 bug (lack
of explode) generally does not break anything, which is why
no tests failed. Still. Good to fix.
The main issue to fix is that questions vesions which should not have
been used (that is, hidden or draft versions) were getting offered
as an option and acutally being used.
As part of this I was able to substantially un-tangle
mod_quiz\question\bank\qbank_helper, which previously was
a mass of functions calling other functions in a complicated way.
Hopefully, it is now a bit easer to understand, and perhaps
less buggy.
This commit will add the index for the question_reference
and the question_set_reference table if its not already
added. This commit will also implement the logic of adding
the question area and component in the joins of the question
reference table in order to make sure any other plugin using
the qbank api does not conflict with each other.
This commit implements a question regrade for the selected
version in the quiz for a slot.

The versioning changes will require some major changes
in the backup and restore of question bank and its
elements. This change introduces those changes to make
it compatible with the new world of versioning in question
bank. This commit also removes quiz_slots fields and
quiz_slot_tags table.
This commit will also introduce the versioning db
structure and some major changes to the quiz
and quiz attempts for the question, random
question and the view.
This commit implements the behat changes for versioning
in core question and associated locations.
This commit adds the changes in questiontype base
to work with new question tables and the new structure in the
databse. Also needed for versioning.
This implementation will also introduct the question status
which allows a question to be in draft and ready status.
I also introduces changes to the base view where it shows
the latest version of the questions. The view of versions
for a question is not implemented in this commit.
This implementation will also introduce changes in the core
qtype plugins to support versioning and the changed
db schema.
Custom fields can now be added to question types.
Site administrators can configure the custom fields and
filed types available. Teachers and question creators can
then use these fields for extra question metadata when
creating questions. Question creators can control if
the fields are displayed to students when they are
taking a quiz or not.
This features uses the core Moodle custom field API.

This implementation will introduce bulk actions qbank
plugins which can grab the selected question and perform
actions according to the selected option from dropdown
in the base view.

This implementation will introduct changes to
the mod_quiz to implement the new qbank api
for view. Major changes are introduced in
the custom view and also all the legacy points
to the old qbank are changed to qbank plugins
where needed. It also deprecated and deletes
classes and scripts which were kept not to
break mod_quiz for the new api implementation.
This commit implements the qbank api so that any plugin
can implement its own question bank. This api currently
works parallely with the moodle core classes and the
added qbank in the core, means the moment a plugin
is installed, that object is replaced with the object
from the plugin instead of core, which means the api
has flexibility till the plugins are integrated and the
plugins can be integrated in any order.
All the old classes are still there and not deprecated
as there is a different tracker for the changes to the
quiz and another tracker for class deprecation and
class renaming. Core question units tests are pointing
to the new api structure but the classes are pointing
to the location related to the plugin availability.

one more array fix
There are several improvements over what we had before:
1. We track all the questions seen in the the student's previous
quiz attempts, so that when they start a new quiz attempt, they get
questions they have not seen before if possible.
2. When there are no more unseen questions, we start repeating, but
always taking from the questions with the fewest attempts so far.
3. A similar logic is applied with variants within one question.
There is lots of credit to go around here. Oleg Sychev's students Alex
Shkarupa, Sergei Bastrykin and Darya Beda all worked on this over
several years, helping to clarify the problem and shape the best
solution. In the end, their various attempts were rewritten into this
final patch by me.
While doing this code clean-up, I noticed that the question bank pop-up was
only showing the question name, whereas it should have been showing the
question name and the start of the question text, so I fixed that.
