MDL-71679 mod_quiz: Updates for new qbank api
...
This implementation will introduct changes to
the mod_quiz to implement the new qbank api
for view. Major changes are introduced in
the custom view and also all the legacy points
to the old qbank are changed to qbank plugins
where needed. It also deprecated and deletes
classes and scripts which were kept not to
break mod_quiz for the new api implementation.

This implementation will introduce a qbank plugin "managecategories"
which will add the question categories feature in the question bank view
by replacing the core classes. Having this plugin will give users
the flexibility of enabling or disabling the category tab.

MDL-71516 core_question: Qbank api implementation
...
This commit implements the qbank api so that any plugin
can implement its own question bank. This api currently
works parallely with the moodle core classes and the
added qbank in the core, means the moment a plugin
is installed, that object is replaced with the object
from the plugin instead of core, which means the api
has flexibility till the plugins are integrated and the
plugins can be integrated in any order.
All the old classes are still there and not deprecated
as there is a different tracker for the changes to the
quiz and another tracker for class deprecation and
class renaming. Core question units tests are pointing
to the new api structure but the classes are pointing
to the location related to the plugin availability.

don't update the value.

Added an upgrade step to remove duplicate stamps, added a unique index
on (contextid, stamp) and modified the question_category edit code such
that stamp duplication in the same context is not permitted.

MDL-48477 questions: no delete icon for only top-level category
...
The back end code already threw an exception if you clicked the icon,
so you could not delete the category, but we should not have shown the
icon in the first place.
Thanks to Pramith Dayananda for working out the fix for this issue. I am
just committing it.

The problem was that array1 + array2 takes the values from
array1 when a key appears in both places.
While working on this area, I updated it to use html_writer.

MDL-24256 question category editing was messed up.
...
The unerlying problem seemed to be too many uses of pass-by-reference in listlib, where it was not necessary.
In investigating this code, I ended up doing a fair bit of cleaning up. Apologies that it leads to an unclear changeset.

MDL-21233 get_query_string() is definitely not recommended in new code, instead use moodle_url constructors with proper parameters; removed some more legacy url concatenation operations; realized out_action is not needed often, because we shoudl pass arounf moodle_url instances instead of old url strings

MDL-21233 escaped parameter is far more importatnt than the overrideparams in out() method - especially all JS urls should be converted to out(false); also it is possible to create new url with overrided parameters in constructor which might be actually work around the double encoding problems in the url (we should never use the out() in moodle_url constructor itself!)
